In medicine, too. This is due to documentation deficiencies, systemic weaknesses in the processes and, last but not least, Human Factors. If the goal is now to minimize errors above the water surface, those below the water must first be reduced.<\/p>\n

In the complex structures of highly divided processes, physicians, nurses and staff in rescue services lack systemic information. Also, leadership cannot always be provided where it is needed – yet decisions must be made all the time.<\/p>\n

They apply not only in aviation, but also in hospitals, emergency medicine and nursing:<\/p>\n

Errors occur, patient safety is no longer a priority.<\/p>\n
Comparable incidents also occur in the hospital, rescue service or emergency medicine. Most physicians, EMTs, and nurses will remember similar situations. After all, work environments are similar. Often, similar error patterns and sources can be identified regardless of the industry. The risks are well known, but they fade into the background in daily practice. Some you may dismiss as truisms. But you must also implement these measures if you want to reduce errors caused by Human Factors. In daily practice, implementation requires hard work on oneself and the team, especially when it comes to the self-evident.[\/et_pb_text][et_pb_blurb title=”Crash of Eastern Airlines Flight 401″ use_icon=”on” font_icon=”%%79%%” _builder_version=”4.9.4″ _module_preset=”1a175db8-d249-440c-bee0-17acb6d9144d”]In the worst case scenario, these twelve Dirty Dozen vulnerabilities can trigger accidents that are basically easily preventable, such as the crash of Eastern Airlines Flight 401 with 75 fatalities about 30 km from Miami Airport into the Everglades.<\/p>\n
The experienced cockpit crew of three had focused on a faulty landing gear indicator and in the darkness had not noticed that they were losing altitude in their holding pattern. The controller, who had noticed the altitude loss, contacted the crew with “Are you okay?” but did not specifically address the altitude loss. By the time the pilots became aware of their misconduct, it was already too late to save the aircraft.<\/p>\n
The entire cockpit crew took care of a critical, but not existential problem, neglecting their main task: flying. Against the background of increasing task complexity on the one hand, and more interfaces and more sophisticated technology on the other, the examination of human factors is gaining steadily in importance, especially in medicine.<\/p>\n
